Connect2Dorset, Dorset Council's managed service recruitment partner, is recruiting an experienced Level 2/3 Qualified Social Worker to join the Preparation for Adulthood Team within the innovative Birth to Settled Adulthood (B2SA) Service.
This rewarding role focuses on supporting children and young people aged 0-25 with autism, learning disabilities, SEND, and complex needs who are at risk of mental health hospital admission. You will work across health, education, and social care services to coordinate support, improve outcomes, and help children and young people remain safely within their communities.
Key Responsibilities
Manage a caseload of children and young people on the Dynamic Support Register (DSR).
Act as the key point of contact for children, young people, families, and carers.
Develop and coordinate person-centred support plans.
Work collaboratively with NHS teams, education providers, and social care professionals.
Attend and contribute to Care, Education and Treatment Reviews (CETRs).
Support hospital admission avoidance and discharge planning.
Provide updates to DSR meetings and multi-agency forums.
Maintain accurate case records, assessments, and reports.
Identify barriers to support and promote joined-up working across services.Essential Requirements
Qualified Social Worker (Level 2/3)
Social Work England Registration
Minimum 2 years post-qualified experience
Experience working with Autism, Learning Disabilities, SEND, Children with Disabilities, or Complex Needs
Experience managing complex caseloads within Children's or Adult Social Care
Strong assessment, care planning, and risk management skills
Knowledge of the Children Act, Care Act, Mental Capacity Act, and SEND Code of Practice
Experience of multi-agency working with health, education, and social care partners
Ability to build positive relationships with children, young people, families, and professionals
Excellent written and verbal communication skills, including report writing
